Enrollment

  1. First access the Print On Demand website. From there, download User Registration form.
  2. Shortly after submission, you will receive a confirmation e-mail with a user ID and password.

The MPI forms are now only available for order through Print on Demand website and are no longer available for order through web/DCS.
